Self-management for people with long term health conditions
About Wandsworth Self-Management Service...

Wandsworth Self-Management Service (WSMS) supports and encourages people with long term conditions in taking a more proactive role in managing their health and wellbeing on a day-to-day basis. This involves an individual learning more about their condition, decision making, taking actions and managing a broad range of factors that contribute to their health.

Wandsworth Self-Management Service also offer a range of free self-management courses for people living with any long term health condition(s) and their carer’s.

Courses available include:

  • Expert Patients Programme (EPP): for people living with or recovering any long-term health condition.

  • Looking After Me (LAM) (for carers): enabling them to develop self-management skills to take control of their own health and wellbeing.

  • New Beginnings (NB): for people living with or recovering from mental health conditions including: stress, anxiety, depression, panic attacks etc.

The main programme is The Expert Patients Programme which offers a tool-kit, of techniques and skills to enable participants to manage their condition better on a daily basis, increase confidence and take control of their condition and its impact on their quality of life.

The programme runs for 2.5 hours, once a week for 6 weeks, with 12-16 people in a group and is led by trained tutors who live with a long-term health condition themselves.

Social prescribing enables primary care (GP’s and nurses) to link patients who present with social, emotional or practical needs to a variety of local non-clinical services. Social prescribing links these patients to support from within the community to promote their wellbeing, encourage social inclusion, promote self-care and offer practical support (e.g. support with housing or debt management etc.).

Can I take part in a course?

Yes, if you...

  • Are over 18

  • Live, work or are registered with a GP in the borough of Wandsworth

  • Have a long term health condition or are a carer

 What will I get out of it?

By the end of the course, you should:

  • Feel more positive and confident

  • Be more in control of your condition

  • Have met like-minded people

  • Have got support and information from other  participants

  • Have been signposted to other resources

  • Have new skills to better manage your condition
