The Wandsworth Care Coordination Centre – one point of contact for all your needs

 

The newly launched Wandsworth Care Coordination Centre is a partnership between Trinity Hospice, Marie Curie and St George’s Healthcare NHS Foundation Trust. The aim of the Centre is to ensure people with a progressive,  life limiting illness can access the services and support they need  from  one place so they can be cared for in their own home or care home.  

 

Who the Centre is for

The Care Coordination Centre is open to any adult with a progressive, life limiting illness living in Wandsworth and to their families and carers.

 

The benefits

The Care Coordination Centre can help you and your family by:

  • providing one point of contact for all your care needs so you don’t need to go to different care providers separately
  • arranging health and personal care support in your home
  • providing you with equipment to support you at home
  • acting as a helpline to give you information, advice and support over the phone.

 

How the service works

The Care Coordination Centre is based at Trinity Hospice and managed by a team of Care Coordinators and a Lead Nurse. They work closely with the professionals involved in your care – such as  your GP, district nurse or hospice – to arrange the services that you need to help you stay at home rather than be admitted to hospital.

Our services

There are a range of services that the Centre can provide, including:
 

  • Care Coordination – we have a team of Care Coordinators who can organise packages of care and act as a central point of communication for you and your family.

     

  • Nursing care – we have access to a dedicated Community Nurse who works with the existing St George’s Community Nursing Team to ensure you can receive the nursing care you need, when you need it.

     

  • Daytime Health and Personal Care  – for a few hours a day a Marie Curie Health and Personal Care Assistant could provide you with that extra bit of support – anything from washing, dressing or loading the washing machine to preparing a simple meal. [Eligibility for this service will depend on your needs and condition.]
     

  • Overnight Nursing Care – we can provide a Marie Curie nurse or healthcare assistant to look after you during the evening and overnight so that your regular carer can take a break.
     

Opening times

The Centre is open on the following days:

Monday to Friday:  7am – 10.30pm

Saturday, Sunday and public holidays:    9am – 5pm

Referrals 

If you would like to be referred to the Care Coordination Centre, please speak to your GP, district nurse or other health professional. Alternatively, you or your carer can contact us directly.
